Severe, recurring headache attacks that occur in clusters or cycles. They cause intense burning or piercing pain on one side of the head, often around the eye. The VA rates cluster headaches using the same criteria as migraines, based on how often attacks happen and how much they affect your ability to work.

Rating Levels

You have very frequent, completely prostrating and prolonged cluster headache attacks that cause severe economic hardship. This is the maximum schedular rating for headaches.

Severe economic impact (job loss, inability to maintain employment)
Multiple medications with poor response
Oxygen therapy or injectable sumatriptan use documented

Common Pitfalls

⚠Not distinguishing cluster headaches from migraines in your medical records — cluster headaches have distinct features (one-sided pain around the eye, tearing, nasal congestion, restlessness) that should be clearly documented by a neurologist.
⚠Not keeping a detailed headache diary — the VA rating is based on frequency and severity of attacks. Without a diary showing dates, duration, and impact, the examiner may undercount your attacks.
