Persistent or recurring blood in the urine that has no identified curable cause. The VA rates chronic hematuria by analogy based on how it affects your voiding, whether it causes anemia, and how much monitoring and treatment you need.

Rating Levels

You have severe or persistent visible blood in the urine requiring medical treatment such as bladder irrigation or cauterization. Your voiding is significantly affected, and you have moderate anemia from ongoing blood loss.

What the VA looks for

Severe gross hematuria requiring bladder irrigation or cauterization
Daytime voiding interval less than 1 hour
Awakening to void 5 or more times per night
Moderate anemia (hemoglobin 8-10 g/dL) from chronic blood loss
Blood transfusions for hematuria-related anemia

Common Pitfalls

⚠Not completing a full hematuria workup — cystoscopy and CT urogram are needed to rule out cancer and establish that the hematuria is chronic and idiopathic. Without this workup, the VA may deny the claim.
⚠Not documenting anemia caused by chronic blood loss — request regular complete blood count (CBC) tests and iron studies. Anemia from hematuria supports a higher rating and may warrant a separate rating.
⚠Not keeping a log of gross hematuria episodes — document every episode of visible blood in the urine including date, severity, and whether you sought medical treatment.
⚠Not connecting hematuria to service — common pathways include in-service kidney injury, exposure to certain chemicals, or secondary to medications prescribed for service-connected conditions.
