A group of bone marrow failure disorders where the bone marrow does not produce enough healthy blood cells. MDS is considered a pre-leukemic condition because it can progress to acute myeloid leukemia (AML). The VA rates MDS based on the severity of cytopenias and the treatment required.

Rating Levels

You have very high-risk MDS requiring a stem cell transplant, or your MDS has transformed to acute leukemia, or you are transfusion-dependent with life-threatening complications.

Stem cell transplant performed or planned
Transformation to acute myeloid leukemia
Transfusion-dependent (multiple units per month)
Recurrent infections from severe neutropenia
Life-threatening hemorrhage from thrombocytopenia

Common Pitfalls

⚠Not connecting MDS to benzene exposure, ionizing radiation, Agent Orange, or other toxic exposures during military service — MDS is strongly associated with environmental exposures common in military settings.
⚠Failing to document every transfusion, hospitalization, and infection — the frequency of these events directly supports higher ratings and demonstrates the severity of your condition.
⚠Not filing for secondary conditions from MDS treatment such as iron overload from transfusions (requiring chelation therapy), infections from low white cells, and fatigue from chronic anemia.
⚠Not understanding that MDS transformation to acute leukemia warrants an immediate increase to 100% — monitor your bone marrow biopsies and file a claim for increase if blast counts rise.
